Job Title: Family Advocate
Department: Case Management
Reports to: Case Management Supervisor
Classification: Nonexempt
JOB SUMMARY: The Family Advocate at Arab American Family Services is responsible for providing support and assistance to individuals and families in need, helping them access resources, and navigating social services. This role involves assessing client needs, developing care plans, and working collaboratively with other service providers to ensure clients receive comprehensive support.
The Family Advocate will assume all other duties as assigned that contribute to the mission, vision, and values of the AAFS.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Conducts assessments of clients’ needs and circumstances.
- Develops and implements individualized care plans.
- Provides guidance and support to clients in accessing community resources and services.
- Advocates on behalf of clients to secure necessary resources and services.
- Maintains accurate and confidential client records.
- Monitors and evaluates client progress, adjusting care plans as needed.
- Collaborates with team members, supervisor and outside social service organizations.
OTHER DUTIES & RESPONSIBILITIES:
- Attends required internal meetings, trainings, and events.
- Other duties as assigned.
EDUCATION AND EXPERIENCE:
Minimum Education
- High school diploma or GED required.
Minimum Experience
- Experience in social services or case management preferred.
KNOWLEDGE, SKILLS AND ABILIITIES:
- Good communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Empathy and understanding towards individuals facing challenging circumstances.
- Organizational skills and attention to detail.
- Comfortable with technology, Microsoft Office experience preferred.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
- Requires sitting for long periods of time.
- Dexterity to perform computerized functions.
- Some local travel for trainings may be required.
